Illnesses and Injuries
Your child’s health and the health of all children are important to us. Healthy children are better learners. No child should be sent to school if he/she has a severe cold, fever, sore throat, vomiting, rash, or any other symptom of a contagious disease (ex. conjunctivitis).
If your child requires an excuse from gym (especially in cases of fracture, sprain, concussion, asthma, etc.) please provide a note from your medical provider.
Administration of Medication in School
Medication (both prescription and over-the-counter) may be administered in school with a written authorization from the medical provider and signed by a parent/guardian. Medication must be brought to the nurse by the parent or guardian in a properly labeled prescription bottle including the name of the student, the name of the medication, dosage, and time of administration. Over-the-counter medications must be brought to the nurse in a new unopened bottle. Middle school students are able to carry and self-administer epinephrine injectors and inhalers with proper authorization from the physician and parent. Medication remaining at the end of the end of the school year must be picked up by the parent.
Physical Examination and Immunization Requirements
Per Connecticut State guidelines, students are required to have physical examinations in the following grades:
- Each year of Pre-K
- Kindergarten entry
- Seventh grade entry
- Out of state transfer
Sports Physicals
All students trying our for, or participating in, sports programs at the Middle School must have an up to date sports physical form on file in the nurse’s office. The sports physical form is required annually and is valid for 13 months from the date it is done. It is the responsibility of the parent and student to get the physical form to the nurse’s office in a timely manner in order for the athlete to be cleared to play.
The “blue” State of Connecticut Health Assessment form must be completed by your child’s physician with all required immunization dates noted. The front of this form is to be completed by the students’ parent or legal guardian. Completed physicals should be turned in to the school nurse.
The state of Connecticut only allows for medical exclusions to immunization requirements. A physician must complete the Student Medical Exemption Certificate for Required Immunization
Health Screenings
The Connecticut State Department of Education mandates that screenings be conducted by your child’s school nurse in the following grades:
- Hearing – grades K, 1st, 3rd, 4th, and 5th
- Vision – grades K, 1st, 3rd, 4th, and 5th
- Postural (scoliosis) – female students in grades 5th and 7th. Male students grade 8
If a referral is necessary, the school nurse will inform the parent/guardian in writing. It is the responsibility of the parent or guardian to schedule the appropriate referral and to provide follow-up information to the school nurse. If assistance is needed to accomplish this, parents/guardians are encouraged to contact the school nurse.
